Why do I have to review all of my information every time I come to my appointment?

What insurance plans do you accept?

We are contracted with most insurance plans. Please check with your plan administrator to verify coverage and co-pay amounts. These amounts can be different from plan to plan even with the same insurance company.

What if I need to cancel my appointment?

We understand that sometimes something may arise which requires you to cancel and reschedule your appointment. We ask that you do so as soon as the need arises and at least 24 hours before our appointment. This will give us the opportunity to schedule another patient who is in need.

What do I need to bring to my first appointment?

Please bring a list of your current medications, including names and dosage, and your Medicare or insurance card.
