What To Expect At Your First Appointment

Please plan to arrive 15 minutes prior to your actual appointment time.  This will give you the opportunity to complete the new patient information form.

Questions on the form include a brief medical history, current health issues and medications you are taking.  It may be helpful for you to bring your prescription bottles with you to this appointment because in addition to the name of the medication, you will be asked the dosage.  Depending on the type of physician you have an appointment with, there will be other questions that are specific to that medical specialty.  You can find a copy of all patient information forms on our web site under the Patient Information tab.

Also please bring your insurance, Medicare, Medicaid or TriCare membership card and a drivers’ license.  We will need to photocopy these for your patient record.
